Healthy at Work / COVID Leave Information

During the 2021-22 school year, the Healthy at Work initiative is to assure staff have a safe, healthy work environment. One result of this initiative is expanded leave options for staff members. The following documents are provided to further explain the changes and options for staff members. 

Please note that as guidance is updated, these documents are updated to meet the latest terminology. 

 

2021-22 HCS COVID Emergency Leave Request Form - Since we have HCS Emergency Leave for COVID-related absences, this leave form will be used instead of our traditional FMLA forms. You can print a copy or can complete via Google here and share with Jinger Carter.

Isolation/Quarantine Decision Tree - This chart covers the various scenarios if you have symptoms, have had close contact with someone with symptoms or if you have been diagnosed with COVID-19. 

 

Employee Leave Decision Tree -     This chart explains the various types of leave now available if an employee needs to be off work. In addition to the COVID-related leave options, there is also traditional sick leave and traditional FMLA. 

What If I am ordered to Isolate/Quarantine due to a household exposure or my positive test?

  • Notify your principal/supervisor immediately 
  • Report your absence to AESOP/Frontline using the appropriate code.
  • Complete and submit the 2021-22 HCS COVID Emergency Leave Request form
  • Submit the appropriate documentation with the leave form to Jinger Carter
